Fractures of the lateral condylar physis     [Back To Top] 

Milch C lassification :

Type I:  

• fracture through the ossification center of the capitellum, lateral to the capitello-trochlear groove

• the elbow is stable and the relationship between the forearm and the humerus remains intact

Type 2:  

• most common

• fractures through the capitello-trochlear groove

• unstable

  Management :

• No displacement < 2mm- immobilise, frequent Xrays as may displace

• Displacement-

  • stage 1 + 2 - MUA (flex and pronate) + percutaneous wires

  • stage 3 - O.R.I.F. lateral approach

• 60 % of lateral condylar physeal injuries need ORIF

Complications

• Lateral condylar overgrowth with spur

• Delayed or non union

• Cubitus valgus (most common)/ varus
